Intake Form
If the individual is currently in a hospital or skilled nursing facility, one of our staff may be able to go out and perform a pre-admission screening. If the individual is at home, copies of medical records relevant to the situation related to the current referral will be requested. Please click here to download the Authorization to Release Medical Information in order to obtain records.

Once the individual case has been screened and CRC is felt to be a good match, the insurance benefits for treatment will be reviewed with the client and/or family. It is up to the individual/family or the referring facility to initiate referrals/authorization as needed, as well as obtaining physician orders for treatment. Once all of this has been obtained, appointments for initial evaluations will be scheduled. Please feel free to set up a time for a visit/tour of the CRC clinic in advance by contacting the clinic supervisor. We accept

Critieria For Admission To NeuroRehab Program
Participants should:
Have diagnosis of TBI/ABI due to trauma, stroke,infectious process, anoxia or other neurologic disease/illness.
Have the ability to benefit from and participate in active therapy and make functinal gains with PT, OT, and/or SLP.
Be adequately medically stable to participate in an outpatient program.
Be behaviorally appropriate such that they are not a risk to themselves or others.
Be free from any current active substance abuse.
Have sufficient endurance to participate in at least 1-2 hours of therapy per day.
Demonstrate a level of functional mobility such that transfers/mobility can be managed by one person and are not more than maximal assistance. If the person requires more assistance, the client must be accompanied by PCA/family participant.
Agree, along with family/significant others, to actively participate and follow through with mutually set goals and plan of care.

OUR MISSION
To provide high quality, individualized, community based rehabilitative and support services to people with neurologic, musculoskeletal or orthopedic injuries or illnesses.
Our program provides an integrated approach to treatment that focuses on successfully transitioning a person back to his or her home, family, school, sports, work, and social spheres.
We pride ourselves on individualized assessments, treatment planning and follow up, all delivered by a caring and sensitive staff in a friendly atmosphere. Patients come for single services or a combination of services customized to match each diagnosis and need.

INDIVIDUAL SUPPORT AND COMMUNITY HABILITAION WORKER
We are currently seeking a part-time employee to work as an Individual Support and Community Habilitation Worker. This person will provide services to individuals with acquired brain injuries to maintain skills that will foster personal growth and independence while living in the community. Services are designed to develop, maintain, and/or maximize the client's independent functioning in self-care, physical and emotional growth, socialization, communication, and vocational skills.
Applicants must be responsible, work independently, have their own reliable transportation, have experience working with individuals with disabilities, and complete required documentation on a computer.
Qualifications: Associate or Bachelor's Degree in health or human services field. COTAs, CTRSs, CNAs, Activity professionals or other healthcare professionals encouraged to apply.

Physical Therapy
We treat a wide variety of functional impairments and disabilities that result from neurologic disease (such as stroke, brain injury, multiple sclerosis, vestibular disease, anoxic brain injury, Parkinson's Disease), as well as orthopedic, post surgical, musculoskeletal disorders and sports injuries.
Licensed physical therapists work directly with patients to provide skilled assessments, with an emphasis on restoring and enhancing physical performance and active healthy lifestyles. The physical therapists have access to athletic club facilities and equipment for exercise programs. The programs place a strong emphasis on patient education to insure a proper understanding of his/her condition and the rationale for the recommended treatment plan. Patients receive customized one-on-one physical therapy services.

Neuropsychology and Social Work Services
We work with highly qualified Neuropsychologists and Social Workers experienced in brain injury post-acute issues to whom we make referrals for short term services. Clinical Neuropsychology is a specialty profession that focuses on brain functioning. Our Social Work services include individual and family counseling, dealing with loss and greif, adjustment to disability and regaining one's place in the family structure.
What is Neuropsychology?
A clinical Neuropsychologist is a licensed psychologist with expertise in how behavior and skills are related to brain structures and systems. In clinical Neuropsychology, brain function is evaluated by objectively testing memory and thinking skills. A very detailed assessment of abilities is done, and the pattern of strengths and weaknesses is used in important health care areas, such as diagnosis and treatment planning. The clinical Neuropsychologist conducts the evaluation and makes recommendations. He or she may also provide treatment, such as cognitive rehabilitation, behavior management or psychotherapy.
How are my test scores used to understand my specific situation?
Your test scores will be compared to scores from people who are like you in important ways. By using database scores from large groups of healthy people for comparison, the Neuropsychologist can judge whether or not your scores are normal for your age and educational background. The pattern of your own test scores will also be reviewed to estimate whether or not there has been a change in certain abilities. How you go about solving the various problems and answering questions during the examination will also be noted. Using these methods, your strengths and weaknesses can be identified.

Community Rehab Care's Case Management Staff have extensive experience in neurological rehabilitation.
SERVICES
Assessment of functional living skills with recommendations and plan development
Ongoing oversight of and adjustment to home-based functional living systems (i.e., memory systems, budgeting, medical appointments, shopping, etc.
Assistance with access to transportation, leisure and recreational resources
Assistance with locating and assessing appropriate housing options
Assistance with obtaining financial resources in the community and with budget issues
Making connections to community based resources
Advocacy and liason with existing service arrangements

Case Management Services funded by the Statewide Head Injury Program are referred directly to Community Rehab Care by the Statewide Head Injury Program.

About Our Clinical and Administrative Leadership
ANN GILLESPIE, MBA, CTRS
Chief Operating Officer
Ann's career spans twenty five years both in Chicago, at Marianjoy Rehab Center, and locally at the Braintree Hospital Rehabilitation Network, both as a practicing clinician and as a manager. The last fifteen years have been devoted to work in the area of Traumatic Brain Injury and the development of the community based brain injury rehabilitation programs. In 1991, Ann received the Professional Merit Award from the Mass Brain Injury Association, served on their Board for four years and continues to remain active on a state and national level. Ann serves on the SHIP (Statewide Head Injury Program) Advisory Board.
EILEEN BROSNAHAN-CHERNOFF, MBA, RN, LicNHA
President of Professional Relations
Former Vice President of Inpatient Services and previously Director of Admissions at the Braintree Rehabilitation Hospital Network for 17 years, Eileen is known for her work in the areas of patient intake systems, pre-admission assessments, and rehabilitation operations. She has been actively involved in many professional associations, including being the Past President of Mass Association of Healthcare Access Management and presently serves on the Steering Committee for Women in Healthcare Management and the Annual Planning Committee for Mass Brain Injury Association.

Clinical Medical Director
Board Certified in Physical Medicine and Rehabilitation
Dr. Glenn serves as an advisor and consultant in helping CRC's management team maintain a high quality of clinical service. Dr. Glenn is the Director of Outpatient and Community Brain Injury Rehabilitation, Spaulding Rehabilitation Hospital, Boston, MA. He was previously the Chair of the Department of Rehabilitation Medicine at Boston University School of Medicine and Director of Rehabilitation Medicine at Boston Medical Center. Along with his other professional affiliations, Dr. Glenn is well known for his numerous presentations and publications, especially in the area of traumaticbrain injury.

COMMUNITY EVENTS
Community Outreach and Recreational Groups
Community Rehab Care assists in the transition from outpatient care to the community through skilled therapy. After discharge from skilled therapy services, Community Rehab Care also provides community outreach though Case Management services, Recreational Groups, and a yearly Snowball event.
Over the years Community Rehab Care staff have been able to establish Recreational Groups for brain injury survivors and program clients. These groups are funded primarily through grants from the Statewide Head Injury Program, obtained by Community Rehab Care.
Community Rehab care employees also take an active role in various events and groups supporting individuals with various neurological diagnoses; including Brain Injury and stroke. Staff assist to lead the B.A.B.I.S. group (Boston Acquired Brain Injury Support Group) and assist in planning the annual Heads Held High walk.

Wellness Program for People with Parkinson's Disease
We offer a collaborative program with Boston University Center for Neuro-rehabilitation to assist those living with Parkinson's Disease to improve overall quality of life through fun exercise!
Our wellness program is designed to help individuals with Parkinson's Disease learn to manage their health more effectively in order to improve day-to-day function and quality of life. Under the guidance of physical, occupational and speech therapists, participants will learn exercises and problem-solving approaches to managing daily mobility, self-care and communication issues.

Wellness Program for People with Parkinson's Disease
Community Aphasia Group
Community Rehab Care has established a program to provide a therapeutic environment for individuals with aphasia to practice their communication skills and socialize each week.
Communication Skills Group
This group will give individualsthe opportunity to practice their communication skills through a variety of modalities (i.e., speaking, writing, gestures). Each week will have a different conversation theme to focus on (music, movies, family, etc.)
Writing Group
This group will provide opportunities for individuals to practice their functional writing skills. Strategies and feedback will be provided. All writing levels accepted!
Lunch and Conversation
A supportive and relaxing environment to practice communication skills. A few outings to local restaurants may be planned throughout the session. Cost of lunch is not included.
