Our Services: Community Case Coordination

Community Rehab Care offers services including Case Management, Community Support Work, Active Treatment. Services are offered through various funding sources including:

Case Management

We offer Case Management services through private pay or through our contracts with MRC’s Statewide Head Injury Program or (SHIP) or through the National Multiple Sclerosis Society’s Case Management Program. Our Case Management services include:

  • Advocacy and liaison with existing service arrangements

  • Assessment of functional living skills with recommendation and plan development

  • Assistance with:

    • Addressing behavior issues

    • Locating and accessing appropriate housing options

    • Planning long-term resources

    • Connecting with medical linkages/support services to address substance issues

    • Securing public entitlements

  • Establishment of connections to community-based resources

  • Ongoing oversight and adjustment of home-based functional living systems such as budgeting, memory systems, medical appointment, shopping, etc.

Community Support Work

We also offer Community Support Work services through private pay or through our contracts with MRC’s Statewide Head Injury Program or (SHIP). Our Community Support Work services include:

  • Accompanying clients to medical and other appointments

  • Assistance with access to transportation, leisure, social, and recreational resources or programs

  • Assistance with managing identified personal care attendant (PCA) services

  • Assistance with monitoring established behavior management programs and appropriate social skills

  • Carrying out recommendations including home exercise programs, meal preparations, and household management activities

ABI/MFP Waiver Services

Community Rehab Care offers multiple services covered by the Massachusetts Home and Community Based Service Waivers (for Persons with Acquired Brain Injury), also known as the ABI/MFP Waivers including:

  • Maintenance-based physical, occupational, and speech-language therapy

  • Individual Support & Community Habilitation
