As part of our commitment to provide quality client-centered services, we collect client satisfaction surveys annually. Each response is analyzed and feedback is used to support program development.
If you participated in therapy services from Community Rehab Care this year and have been discharged, we encourage you to let us know how we did by submitting a survey!
(Passwords are provided by the therapy team to each client upon discharge)
